PROCEDURES & FORMS

Please read the directions carefully. Pay close attention to highlighted directions.

Conference Request Form

The Conference Request Form should be filled out each time you wish to attend a Conference, Workshop, or Meeting.  You should also fill out a form if you are participating in a Visitation for school purposes.  A Conference or Workshop flier or brochure should be attached to the Conference Request Form.  This form is necessary to allow us to hire and pay a substitute teacher.  Please follow the directions below:

Exceptions:

·         If you are attending a workshop or conference that occurs outside of the contractual day, and which you are anticipating an eventual salary increase, please submit a Request for Prior Approval – In-Service Credit Form.  You do not need to submit a Conference Request Form in this situation.

·         If you are attending an in-house after work hours in-service workshop, the instructor will provide you with a form to use for salary credit.  You do not need to submit a Conference Request Form in this situation.  You can eventually submit this form with an In-Service Credit Form for a salary increase.

Directions for Submitting a Conference Request Form

Please submit the Conference Request Form, along with all appropriate paperwork, to your Curriculum Coordinator.  The request, if approved, will be forwarded to the following:  Building Principal/Supervisor, Superintendent of Schools, and then Assistant Superintendent.

1.   If there is no expense associated with the request (i.e. Cayuga-Onondaga BOCES) and the request is approved:

  • You will receive an e-mail confirming the approval from the Superintendent’s Secretary
  • You need to complete the registration for the workshop
  • You will need to follow the normal procedure for your building for making arrangements for a substitute

2.   If a cross-contract is required (a conference or workshop at a BOCES other than the Cayuga-Onondaga BOCES), that paperwork will be generated in the Superintendent’s Office.  If the request is approved:

  • You will receive an e-mail confirming the approval from the Superintendent’s Secretary
  • You need to complete the registration for the workshop
  • You will need to follow the normal procedure for your building for making arrangements for a substitute

3.   If there are costs associated with the request:

A.    if the district is paying for the conference or workshop, a Requisition Form (This form is now on the web page) must be attached to the Conference Request Form along with any additional documentation (flier or brochure)

·         The District will register you for the workshop.

·         You will receive an e-mail confirming the approval from the Superintendent’s Secretary

·         You will need to follow the normal procedure for your building for making arrangements for a substitute

Or

B.     if the staff member is paying directly for the conference or workshop and would like to receive reimbursement, a Requisition Form needs to be submitted.  All anticipated expenses need to be listed (some may have to be approximated), including registration fees, hotel costs, meals, and mileage.  Upon return from the conference or workshop, the staff member needs to submit an Expense Reimbursement Form along with the copy of the Requisition Form that was returned to you.  All receipts must be included to be reimbursed for any expenses that the staff member incurred.  If mileage is being requested, please include a Mapquest printout for distance verification.  If tolls are paid, receipts are required for reimbursement.

·         You need to register yourself for the workshop.

·         You will receive an e-mail confirming the approval from the Superintendent’s Secretary

·         You will need to follow the normal procedure for your building for making arrangements for a substitute

Expense Reimbursement Form

This form needs to be filled out whenever a staff member pays directly for costs associated with a conference or workshop.  It is submitted with a copy of the Requisition Form (which should have been submitted with the initial Conference Request Form).  Receipts must be attached to the Expense Reimbursement Form in order to receive reimbursement for out-of-pocket expenses (including mileage – your receipt can be a Mapquest print-out).
